_________I understand that I am not PURCHASING a pet but ADOPTING and SAVING THE LIFE of an orphaned animal, therefore there is NO TRIAL PERIOD and will be NO refund for ANY reason other than the pet being terminally ill (and this is confirmed by one of our vets).

The dog or puppy will be left alone for __________hours.

_________This animal has an affectionate and friendly manner and the rescue will not place a animal unknown to have an unsound temperament. However, no one can predict how any animal will react in given situation, especially without complete knowledge of its life experiences. Therefore, adoptive owners are strongly urged to exercise prudence and caution in introducing an adoptive animal  into a new situation until the animal  has become fully adjusted to its new environment and the adoptive owner has had an opportunity to become familiar with this animal's unique personality.

________Small children should be supervised carefully around all animals and taught the proper ways in which to play with and handle a animal. The animal  should not be viewed as a "toy" at any time.

________This animal  must never be allowed to run loose. He/she should always carry identifying tags or have a microchip implant. Adopters must agree to always walk the dog on a leash, unless an enclosed, secure yard is accessible.

________This animal  is to be kept as house animal  in close companionship with his family. He/she should also be in a home where he will be sleeping in the home and not left outdoors. is never to be tied outdoors for any length of time.

________ Adoptive owners must be prepared to demonstrate their ability and willingness to provide proper food, medical care and a loving, safe, healthy and clean environment for this animal.

_________The ownership and responsibility for this animal belongs solely to the adoptive owners.However, if for some valid reason, the adoption does not work to the advantage of the new owner and the animals, New Beginnings Dog Rescue must be contacted and given the option of re-acquiring the animal  before any further placement.

_________ Each prospective adoptive home may be visited by a Representative of New Beginnings Dog Rescue to evaluate its qualifications as an adoptive home.

__________ New Beginnings Dog Rescue makes no representations or warranties regarding the temperament or physical condition of the adoptive animal.

The Rescue shall not be responsible for any bodily injuries or property damage caused by any adopted animal or by the actions of its adoptive owners. The adoptive owner hereby specifically assumes sole responsibility for and agrees to hold the Rescue harmless from, any and all loss and expenses (including legal fees).
